5.PERFORMANCE ENHANCHING DRUGS BANNED IN SPORT

INTRUDUCTION

SEE BELOW FOR AN EXTRACT OF THE LIST OF BANNED DRUGS AS DETAILED BY THE IOC AS ON 01 APRIL 2000. IT IS VALID UNTIL FURTHER NOTICE. THE LIST REPRESENTS DIFFERENT DOPING CLASSES TO ILLUSTRATE THE DOPING DEFINITION. NO SUBSTANCES BELONGING TO THE BANNED CLASSES MAY BE USED EVEN IF THEY ARE NOT LISTED AS EXAMPLES. FOR THIS REASON, THE TERM "AND RELATED SUBSTANCES" IS INTRODUCED. THIS TERM DESCRIBES DRUGS THAT ARE RELATED TO THE CLASS BY THEIR PHARMACOLOGICAL ACTIONS OR CHEMICAL STRUCTURE. HIGHLIGHTED COPY REFERS TO INGREDIENTS WHERE USE IS SUBJECT TO CERTAIN RESTRICTIONS.
I. DOPING CLASSES
A. STIMULANTS*

• BETA-2-AGONISTS, EG FORMOTEROL, SALME-TEROL, SALBUTAMOL AND TERBUTALINE (PERMIT¬TED ONLY AS INHALER AND MUST BE DECLARED TO THE RELEVANT MEDICAL AUTHORITY)

• CAFFEINE - IF THE CONCENTRATION IN THE URINE EXCEEDS 12 PG/ML

• IMIDAZOLE PREPARATIONS FOR TOPICAL USE ARE ACCEPTABLE, EG OXYMETAZOLINE. TOPICAL (IE. OPHTHALMOLOGICAL AND NA-SAL APPLICATION OF PHENYLEPHRINE IS PERMITTED). VASOCONSTRICTORS SUCH AS ADRENALINE MAY BE ADMINISTERED WITH LOCAL ANAESTHETIC AGENTS.

C. ANABOLIC AGENTS
• ANABOLIC ANDROGENIC STEROIDS

THE PRESENCE OF A TESTOSTERONE (T) TO EPITESTOSTERONE (E) RATIO GREATER THAN SIX (6) TO ONE (1) IN THE URINE OF A COMPETITOR CONSTITUTES AN OFFENCE UNLESS THERE IS EVIDENCE THAT THIS RATIO IS DUE TO A PHYSI¬OLOGICAL OR PATHOLOGICAL CONDITION, E.G. LOW EPITESTOSTERONE EXCRETION, ANDROGEN PRODUCING TUMOUR, ENZYME DEFICIENCIES.

"INSULIN IS PERMITTED ONLY TO TREAT INSULIN-DEPENDENT DIABETES. WRITTEN NOTIFICATION OF INSULIN-DEPENDENT DIA¬BETES BY AN ENDOCRINOLOGIST OR TEAM PHYSICIAN IS NECESSARY.
THE PRESENCE OF AN ABNORMAL CON¬CENTRATION OF AN ENDOGENOUS HORMONE OR ITS DIAGNOSTIC MARKER(S) IN THE URINE OF COMPETITOR CONSTITUTES AN OFFENCE UNLESS IT HAS BEEN CONCLUSIVELY DOCU¬MENTED TO BE SOLELY DUE TO A PHYSI¬OLOGICAL OR PATHOLOGICAL CONDITION.
II. PROHIBITED METHODS
A. BLOOD DOPING: MEANS THE ADMINISTRATION
OF BLOOD, RED BLOOD CELLS AND/OR RELATED
BLOOD PRODUCTS TO AN ATHLETE, WHCIH MAY
BE PRECEDED BY WITHDRAWAL OF BLOOD
FROM THE ATHLETE, WHO CONTINUES TO TRAIN
IN SUCH A BLOOD-DEPLETED STATE.
B. ADMINISTERING ARTIFICIAL OXYGEN CAR-
RIERS OR PLASMA EXPANDERS
C. PHARMACOLOGICAL, CHEMICAL AND
PHYSICAL MANIPULATION
III. CLASSES OF PROHIBITED SUBSTANCES IN CERTAIN CIRCUMSTANCES
POSITIVE TESTS WHICH MAY LEAD TO SANCTIONS
A. ALCOHOL
WHERE THE RULES OF A RESPONSIBLE AUTHORITY SO PROVIDE,TEST WILL BE CONDUCTED FOR ETHANOL.
B. CANNABINOIDS
WHERE THE RULES OF A RESPONSIBLE AUTHORITY SO PROVIDE, TESTS WILL BE CONDUCTED FOR CANNABINOIDS (E.G. MARIJUANA, HASHISH). AT THE OLYMPIC GAMES, TESTS WILL BE CONDUCTED FOR CANNABINOIDS. A CONCENTRATION IN URINE OF 11-NOR-DELTA 9-TETRAHYDROCANNABINOL-9-CARBOXY-LIC ACID (CARBOXY-THC) GREATER THAN 15 NANO¬GRAMS PER MILLIMETRE CONSTITUTES DOPING.
C. LOCAL ANAESTHETICS
INJECTABLES ARE PERMITTED ONLY UNDER THE FOLLOWING CONDITIONS:
• BUPIVACAINE, LIGNOCAINE, MEPIVA-CAINE, PROCAINE, ETC, BUT NOT CO¬CAINE. VASOCONSTRICTOR AGENTS (E.G. ADRENALINE) MAY BE USED IN CONJUNC¬TION WITH LOCAL ANAESTHETICS.
• ONLY LOCAL OR INTRA-ARTICULAR INJECTIONS
ONLY WHEN MEDICALLY JUSTIFIED. DETAILS INCLUDING DIAGNOSIS, DOSE AND ROUTE OF ADMINISTRATION MUST BE SUBMITTED PRIOR TO THE COMPETITION OR, IF ADMIN¬ISTERED DURING THE COMPETITION, IMME¬DIATELY AFTER INJECTION, IN WRITING TO THE RELEVANT MEDICAL AUTHORITY.
D. GLUCO-CORTICOSTEROIDS
THE SYSTEMIC USE OF GLUCOCORTICO-STEROIDS IS PROHIBITED WHEN ADMINIS¬TERED ORALLY, RECTALLY, OR BY INTRAVENOUS OR INTRAMUSCULAR INJECTION.
E. BETA-BLOCKERS
• ACEBUTALOL, WHERE THE RULES OF A RE¬SPONSIBLE AUTHORITY SO PROVIDE
• ATENOLOL
• LABETALOL
• OXPRENOLOL
• METOPROLOL

IV. OUT OF COMPETITION TESTING
UNLESS SPECIFICALLY REQUESTED BY THE RESPONSIBLE AUTHORITY,OUT OF COMPETITION TESTING IS DIRECTED SOLEY AT PROHIBITED SUBSTANCES IN CLASS I.C.(ANABOLIC AGENTS),I.D.(DIURETICS),I.E.(PEPTIDE HORMONES,MIMETICS AND ANALOGUES),AND II(PROHIBITED METHODS)


CAUTIONARY NOTES:
• NEW MEDICINES ARE CONTINUALLY BEING DEVELOPED AND LAUNCHED, THEREFORE NO LIST IS EVER COMPLETE.
• THE ONUS RESTS ON THE PATIENT OR PRACTITIONER TO CHECK WITH A GIVEN SPORTING BODY ON WHETHER A SPECIFIC INGREDIENT IS BANNED OR IF RESTRICTIONS ARE APPLICABLE IN THAT GIVEN SPORT PRIOR TO USE FOR CONDITIONS THAT ARE NOT LIFE-THREATENING.
